We do not make any guarantees regarding specific outcomes or results from physical therapy sessions. Success in physical therapy is dependent on a variety of factors, including individual health, commitment to the treatment plan, and adherence to any prescribed exercises or recommendations.

Responsibility for Health and Safety

By participating in physical therapy with Move Well PT, you acknowledge that you are in good health and have consulted with a licensed healthcare provider if necessary prior to beginning any physical therapy treatment. You are responsible for notifying your therapist about any changes to your medical history, condition, or health status. If you experience any pain, discomfort, or complications during your treatment, it is your responsibility to immediately inform your therapist.

Payment and Fees

As a cash-only, mobile physical therapy practice, all payments for services are due at the time of service. Move Well PT reserves the right to update or change the payment and cancellation policies at any time. Any such changes will be communicated to clients in advance.

Limitation of Liability

By using the services of Move Well PT, you agree to indemnify and hold harmless the Company, its therapists, employees, and agents from any claims, losses, damages, or liabilities arising out of your participation in physical therapy services. Move Well PT is not responsible for any accidents, injuries, or damages that may occur before, during, or after a physical therapy session.

Confidentiality

Your privacy is important to us. We take all reasonable measures to protect the confidentiality of your personal and health information. Any information shared during your physical therapy sessions will be kept confidential and used solely for the purpose of your treatment. However, we may disclose information as required by law or with your express consent.

Mobile Services Disclaimer

As a mobile practice, Move Well PT provides in-home physical therapy services. By agreeing to our services, you understand and accept that the quality of care may be influenced by the location and environment of the appointment. You agree to provide a safe and suitable location for therapy sessions and acknowledge that Move Well PT cannot be held liable for any incidents or accidents that may occur on the premises where services are rendered.
